1 W-9 Form

This ensures we have the correct tax information on file. You may download the form here. Once you've completed and signed the form, download it to your computer. It should not be sent to the IRS, only to KLEENUP.

Why You Need Insurance

As a KLEENUP cleaner, you are an independent contractor, not KLEENUP’s employee. This means you are responsible for your own insurance coverage. KLEENUP does not provide insurance for you or your employees. To bid on projects, you must have the required insurance policies and submit a Certificate of Insurance (COI) verifying your coverage.

Required Insurance Coverage

  • General Liability Insurance
    Protects you against property damage or injuries caused while performing a job
  • Worker’s Compensation Insurance (if applicable)

    This is only required if you hire employees.

  • Auto Liability Insurance (if applicable)

    • If you use a personal vehicle for work (e.g. traveling to job locations, transporting cleaning supplies, etc.), you must confirm that your vehicle is insured under a personal or commercial policy.
    • If you use a commercial vehicle, it must be insured under a commercial auto policy.
    • No vehicle? You may skip this requirement.

No Insurance Yet?

If you don’t have insurance yet, you can choose one of the following options to find the right coverage for your needs.

  • Find your own provider: Contact a local insurance agent or use online platforms like Hiscox or Next Insurance.
  • Work with an independent agent: While not affiliated with us, our recommended agent can assist you. Simply download the application form here, fill it out with your information and send it to onboarding@kleenup.com. We’ll forward it to the agent, who will then reach out to you with a quote.

How to Get a Certificate of Insurance (COI)

  1. Download the requirements here. This document details the required insurance coverage to meet the limits specified in the Terms of Use Agreement.
  2. Send the downloaded file to your insurance provider and request a Certificate of Insurance (COI).
  3. Have your insurance agent email the completed COI directly to onboarding@kleenup.com on your behalf.

If you're a self-employed contractor in Maryland with no employees, please complete the Sole Proprietor's Status Form. You may download the form here. Make sure to select Option 2 ("I have NOT elected to become a covered employee").

All KLEENUP cleaners are required to complete a brief Background Authorization Form to allow for a routine background check. You may download the form here. This process helps ensure a safe, secure community for our clients and cleaning professionals, while upholding our high standards of trust and professionalism. Your information will be handled securely and confidentially.

Reply to the onboarding email where you received the link to this page, attaching the following:

  1. W-9 form
  2. Sole Proprietor's Status form (if applicable)
  3. Background Check Authorization form
